BACKGROUND:
The Vaughan Community Health Centre (VCHC) is a not-for-profit, community-governed organization providing primary health care, social services, and illness prevention programs primarily to the residents of the Western and Northern York Region. We serve people experiencing barriers to accessing health care services due to culture, language, age, chronic illness, or the like. We are committed to providing services with and for local Indigenous, Black, racialized and other disadvantaged communities. Family physicians from racialized communities are welcome to apply.

The Family Physician will work within a team-based care environment consisting of 4 Family Physicians, 3 Nurse Practitioners, 2 RPNs, a phlebotomist, and access to interprofessional health providers such as system navigators, chiropodists, physiotherapists, social workers, community dietitians, and chronic disease management team. Client care is provided in-person and virtually. We provide a flexible work schedule and work-life balance.

POSITION SUMMARY

As a member of the interdisciplinary team, the Family Physician provides primary care to clients and their families within a care model which emphasizes health promotion, disease prevention, and client education. The Family Physician will deliver culturally and linguistically responsive and safe care that meets the needs of local population, including underserved communities (e.g. Indigenous, Black, 2SLGBTQIA+, etc.).

KEY ACCOUNTABILITIES AND RESPONSIBILITIES
- Provide primary care to individuals of all ages and their families by considering risk factors (environmental, social, psychological and medical) in a manner which can be understood by the client, and when applicable, their families.
- Perform clinical duties, which include but are not limited to history-taking, advanced physical assessments, diagnostic reasoning and development of treatment plans which also include health promotion/ education/ counselling, screening, referral, and follow-up.
- Provide services during regular and extended hours on a scheduled basis and on an urgent basis in a variety of settings such as within the clinic, in community locations, and home visits as required. Possible sharing of on-call duties with other providers.
- Coordinate or be involved with clients during periods of hospitalization, or with community health professionals when clients are receiving in-home healthcare.
- Work within a healthcare framework which emphasizes health promotion and education, disease prevention, and recognize the special risk factors (social, physical, psychological, etc.) which may affect the health of clients.
- Ensure appropriate, comprehensive treatment is delivered by maintaining complete and accurate medical records, participating in chart reviews and discussion of problem cases, and answering client inquiries and directing them to or providing appropriate treatment.
- Implement quality improvement strategies to improve client services and promote more efficient team functioning and workflows.
- Utilize primary care metrics, including client experience results, outcome measures and information about perceived needs obtained through client encounters to develop responsive interventions for client care.
- Ensure panel size and key performance indicators identified by the organization and funder are met.
- Work in partnership with other members of the primary care team to share responsibility for planning, provision and coordination of client care.
- Undertake teaching and training of medical and nursing students when required.

QUALIFICATION REQUIREMENTS
- Medical degree and family medicine residency from a recognized university.
- CCFP certification.
- Active registration and good standing with the Ontario College of Physicians and Surgeons.
- Proof of Canadian Medical Protective Association coverage.
- Current Cardiopulmonary Resuscitation (CPR), Level C
- Two to five years of experience including residency in primary care settings (community-based preferred).
- Experience working with all ages in the role of Family Physician in a primary care setting.
- Proficient in using electronic medical records (EMR) systems and other healthcare technology commonly used in Ontario's primary care setting.
- Excellent interpersonal, written and oral communication skills.
- Demonstrated ability to work effectively in a multi-disciplinary team environment.
- Experience in delivering culturally and linguistically responsive and safe care that meets the needs of local population, including underserved communities (e.g. Indigenous, Black, 2SLGBTQIA+, etc.).
- Proficient in a second language that reflects the diversity of the communities served by VCHC is an asset.
- Commitment to ongoing professional development and staying current with best practices.
- Ability to prioritize, manage time effectively and be flexible in a very active work environment.
- Valid driver's license, regular access to a vehicle and appropriate insurance for personal automobile.

Job Description
As a family physician practicing in Canada, you will play a critical role in delivering comprehensive, patient-centered healthcare. Your duties include but are not limited to the following:

Primary Care and Patient Management
Diagnosing and Treating Illnesses: Assess, diagnose, and manage a wide range of acute and chronic medical conditions in patients of all ages.

Applicants must be eligible for an academic appointment within the Department of Obstetrics & Gynecology at the University of Ottawa. A strong clinical background in general obstetrics/gynecology and/or demonstrable experience and excellence in teaching, research, quality initiatives, or clinical care would be highly valued. Expertise in Minimally Invasive Gynecologic Surgery, particularly with advanced ultrasound skills in the management of Endometriosis, would be a valuable addition to our existing clinical programs. While not required, additional training in a relevant Master program would be considered an asset to the Department (e.g. Public Health, Health Care Management, Epidemiology, or Medical Education).

The position involves clinical work, including on call duties, and teaching of all levels of trainees including medical students, residents, and fellows. All faculty members can support our Department's robust research program through collaboration and involvement in patient recruitment. The appointee should be a team player, be able to interact effectively with patients, colleagues, staff and management, and function well in a high-volume environment with a commitment to provision of high-quality patient and family centered care.
