Cloverdale Medical Centre is a community-based medical clinic located inside Cloverdale Mall in Etobicoke, Ontario. We provide a wide range of primary care and walk-in services to a diverse patient population. The clinic is fully equipped and supported by experienced administrative staff in a collaborative and patient-focused environment.

Position Summary

We are looking for two (2) permanent Family Physicians to provide family medicine services to the community on a walk-in basis and by appointment. The Family Physician will be responsible for delivering comprehensive primary care services in accordance with the highest standards of medical practice and regulations set by the College of Physicians and Surgeons of Ontario.

Specific duties include:

- Providing primary and continuing medical care to patients of all ages.
- Diagnosing and treating a wide range of medical conditions, including acute illnesses, chronic diseases, physiological disorders, and minor injuries.
- Conducting physical exams, routine health assessments, and managing chronic conditions.
- Ordering and interpreting laboratory tests, diagnostic imaging, and other relevant investigations.
- Prescribing and administering medications, treatments, and performing minor procedures, as necessary.
- Delivering preventive care, including immunizations, health education, and lifestyle counseling.
- Providing urgent care for minor emergencies and acute presentations.
- Maintaining accurate and up-to-date patient records in accordance with provincial and clinical standards.
- Participating in care coordination and referrals to specialists when appropriate.

The Family Physician will be expected to work full-time (at least 32 hours per week) at our practice located at 174B - 250 The East Mall, Etobicoke, Ontario, M9B 3Y8. The Family Physician will be permitted to customize their own clinic hours, including working additional hours, evenings and weekends.

Qualifications/Requirements

The following are required:

- Medical Degree
- Must be eligible for licensure with the College of Physicians and Surgeons of Ontario and the Ontario Medical Association.
- Prior to starting practice, required to obtain medicolegal insurance with the Canadian Medical Protective Association, or with another carrier at your expense.
- Letter of good standing from previous employer.
- Must provide Criminal record check, Child and Adult abuse check prior to starting practice.
- At least a 3-year Return of Service Agreement is required, with the option of an agreement up to 5-years.
- All duties will be performed in English.

Compensation and Benefits

Compensation will be fee-for-service, in accordance with the physician's billing under the Ontario Health Insurance Plan (OHIP), in accordance with the Schedule of Benefits and policies established by the Ontario Ministry of Health. Anticipated total annual compensation based on this model, and our practice size, is approximately CAD$300,000 to CAD$400,000 annually.

VCHC has implemented a direct referral system to improve access to inter-professional care by allowing non-team-based family physicians to connect vulnerable (medically and socially complex) clients to services available at the VCHC. As part of the health promotion framework of our services, we work in partnership with other community-based health and social service organizations to address individual, family and community needs.



POSITION SUMMARY:
As a member of the interdisciplinary team, the Family Physician provides primary care to clients and their families within a care model which emphasizes health promotion, disease prevention, and client education. The Family Physician will deliver culturally and linguistically responsive and safe care that meets the needs of local population, including underserved communities (e.g. Indigenous, Black, 2SLGBTQIA+, etc.).

KEY ACCOUNTABILITIES AND RESPONSIBILITIES:
Provide primary care to individuals of all ages and their families by considering risk factors (environmental, social, psychological and medical) in a manner which can be understood by the client, and when applicable, their families.

Perform clinical duties, which include but are not limited to history-taking, advanced physical assessments, diagnostic reasoning and development of treatment plans which also include health promotion/ education/ counselling, screening, referral, and follow-up.

Provide services during regular and extended hours on a scheduled basis and on an urgent basis in a variety of settings such as within the clinic, in community locations, and home visits as required. Sharing on-call duties with other providers.

Coordinate or be involved with clients during periods of hospitalization, or with community health professionals when clients are receiving in-home healthcare.

Work within a healthcare framework which emphasizes health promotion and education, disease prevention, and recognize the special risk factors (social, physical, psychological, etc.) which may affect the health of clients.

Ensure appropriate, comprehensive treatment is delivered by maintaining complete and accurate medical records, participating in chart reviews and discussion of problem cases, and answering client inquiries and directing them to or providing appropriate treatment.

Implement quality improvement strategies to improve client services and promote more efficient team functioning and workflows.

Utilize primary care metrics, including client experience results, outcome measures and information about perceived needs obtained through client encounters to develop responsive interventions for client care.

Ensure panel size and key performance indicators identified by the organization and funder are met.

Work in partnership with other members of the primary care team to share responsibility for planning, provision and coordination of client care.

Undertake teaching and training of medical and nursing students when required.

QUALIFICATION REQUIREMENTS:
Medical degree and family medicine residency from a recognized university.

CCFP certification.

Active registration and good standing with the Ontario College of Physicians and Surgeons.

Proof of Canadian Medical Protective Association coverage.

Current Cardiopulmonary Resuscitation (CPR), Level C

Two to five years of experience including residency in primary care settings (community-based preferred).
