Position Statement/Summary
As a member of the inter-professional primary health care team, you will provide comprehensive primary health care with a focus on OB-GYN and health promotion to clients of all ages, on and off-site, including home visits; work independently and as part of an inter-professional team, and participate in on-call services. You will also be involved in organizational, team-based or individual quality and performance improvement projects and initiatives that improve client outcome and increase access to care.
Key Responsibilities
Family Medicine
- Provides ongoing client-centered, comprehensive primary medical care to clients of all ages, including assessment, diagnosis, treatment and follow-up, counselling, screening, referral, and client education.
- Provides comprehensive primary medical care to populations facing multiple barriers, ensuring access to those most vulnerable.
- Provide itinerant services working from different locations to create greater access to populations facing barriers to care.
- Participate in the rotation for on-call among physicians, providing advice to patients (in-person, via phone or other virtual platforms), and dealing with urgent lab results.
- Collaborates with other members of the inter-professional team; and consults as required with Nurse Practitioners to enable them to work with their full scope of practice.
- Sees clients primarily on site - on both scheduled and urgent basis - and as needed in the community through outreach, off-site clinics, home and hospital visits.
- Participates in primary health care programs and service delivery in partnership with other institutions and agencies as required.
- Initiates, participates in, or leads appropriate in-service training sessions.
- Ensures that infection control policies and procedures are followed.
- Fosters clients' self-help, mutual aid and capacity building by assisting with the development, implementation and evaluation of health promotion programs that address the needs of the community, as appropriate.
- Where appropriate, develop partnerships with other agencies to improve health care service provision and better address the needs of the community.
- Maintains complete and accurate client records, assessment notes and required correspondence.
- Safeguards client records and assures confidentiality and privacy of client information.
- Participates in the implementation of the organizational quality improvement initiatives and works towards achieving organizational goals to ensure access to primary health care services.
- Contributes to programs and services cycle of planning and evaluation, and participates in center-wide planning, committees, and team meetings as appropriate.
- Participates in inter-professional research with colleagues - internal and external to the Centre, as appropriate.
- Maintains all licenses, certificates, and standards related to the ability to practice as an MD in Ontario.
FMOB/ Low risk obstetrics (asset to the role)
- Provide comprehensive care for obstetrical clients (prenatal, intrapartum, postpartum), including care to populations facing multiple barriers ensuring access to those most vulnerable.
- Run the shared prenatal clinic (alternates between our 2 FMOB providers)
- Become a member of the FMOB team at St-Joseph's Health Centre and participate in the shared FMOB call schedule
- Offer support and internal consultations in FMOB to the other primary care providers.
- Offer contraception, STI management, and preventative care.
- Offer contraceptive procedures: IUD insertion and removal, Nexplanon insertion and removal
- Collaborate internally with the interprofessional team, and with external family physicians, OB-GYNs and other specialists.
- Maintain accurate EMR documentation and meet quality metrics.
Qualifications
- Degree in medicine from a recognized school or faculty of medicine.
- Current license to practice medicine from the College of Physicians and Surgeons of Ontario.
- Successful completion of a two-year post graduate residency program accredited by the College of Family Physicians of Canada and an active member of the College.
- Experience providing comprehensive primary medical care to populations facing multiple barriers and with comorbidities, including mental health and addictions.
- Strong clinical skills.
- For FMOB applicants: PGY3 Enhanced Skills Program in family low risk obstetrics encouraged.
- High-level clinical competency, leadership, patient safety advocacy, adaptability, program planning awareness.
- Demonstrated ability, commitment to and knowledge of community health care.
- Demonstrated ability to work in a team-based model and within an inter-professional team environment.
- Excellent interpersonal skills with clients in a culturally diverse practice population.
- Excellent communication, empathy, decision-making, problem-solving, conflict management and time management skills.
- Proficiency in the use of computers, Microsoft Office applications and electronic health records.
- Second language, relevant to catchment area populations, is an asset.
